• This website, created by ProPublica, reports the total of track ratings from the last 3 years to find the severity of deficiencies. 

• Deficiencies are given when the nursing home facility fails to meet specific requirements which are identified by the letter A (the least serious deficiency) to the letter L (the most serious deficiency).

• The tracker can help you choose the right place for you to live.

• Each facility is surveyed by different teams. Some are more specific and may be stricter, giving great facilities a bad deficiency. 

• In searching of a facility, you must give the exact name of the nursing facility that the company goes by, especially since some of them are under the umbrella of a larger company.
